What the request form collects
The forms ask for your name and email, and optionally your phone number, your ZIP code or neighborhood, what you are interested in, when you are hoping to have a consultation, and a short message. The forms do not ask for your medical history, medicines, conditions, photos, age or date of birth. Please do not send them: keep them for the consultation.
Your consent
Every form has a consent box that is never ticked in advance. By ticking it you agree that Breast Augmentation NYC may share your request with one or more independent providers in your area so they can contact you by email or phone about it. A record of your consent, with the date and time, is kept with the request.
Who your request is shared with
Your request is shared only with independent plastic surgery offices in the New York City area that can answer it, and with the service providers that run this website and its email. It is not sold to anyone for any other purpose. Once an office has your request, its own privacy practices apply to what it does with it.
Cookies and site statistics
The site may use basic cookies and visit statistics to keep pages working and to count visits. These do not identify you by name, and your form details are not passed to advertising tools.
How long details are kept
Requests and consent records are kept only as long as needed to pass them on and to show that consent was given, and then deleted.
Your choices
You can ask to see the details held about you, to correct them or to have them deleted, and you can withdraw your consent for future sharing at any time. Send your request through the contact page and write “privacy” in the message. Withdrawing consent does not undo sharing that has already happened, so contact any office that has already replied to you as well.
Children
This site is meant for adults. It does not knowingly collect details from anyone under 18.
Changes to this policy
If this policy changes, the new version is posted on this page with a new date.
