The decisions

Six choices inside one operation

Breast augmentation is not one decision. Knowing these six first turns the consultation into a real conversation.

  1. What fills it

Silicone gel, saline, or your own fat in place of an implant.

    • Silicone gel
    • Saline
    • Structured saline
    • Your own fat

  2. Surface

Smooth or textured. The FDA says BIA-ALCL “appears more often in patients with textured breast implants.”

    • Smooth
    • Textured
  3. With a lift or without

ASPS notes implants do not correct significant sagging, which may need a lift.

    • Implants alone
    • Implants and a lift
    • A lift alone

3 more choices
  1. Shape

Round or shaped. ASPS notes shaped implants can rotate and may need a longer incision.

    • Round
    • Shaped
  2. Incision

Where the implant goes in, and so where the scar sits.

    • Breast fold
    • Areola
    • Armpit
    • Navel
  3. Size and placement

ASPS lists both among the choices a surgeon should explain. Ask why they suggest what they do.

    • Size
    • Placement

Silicone, saline or your own fat?

Three ways to add volume. Only a surgeon who examines you can say which suit you.

Silicone gelSalineYour own fat
What it isA shell filled with silicone gel, which ASPS says feels closer to natural tissue.A shell filled with sterile salt water.Fat taken by liposuction and injected. No implant.
If it leaksThe gel may stay put: a silent rupture, found by MRI or ultrasound. It collapses and the body absorbs the saline.Nothing to leak, but some fat does not survive.

What goes into a quote

A quote has several parts. Ask for each one in writing, so two quotes can be compared fairly.

  1. SurgeonThe surgeon’s own fee
  2. AnesthesiaThe anesthesia provider
  3. FacilityThe operating room
  4. ImplantsThe devices themselves
  5. GarmentsSupport bras after surgery
  6. Tests and medicinesTests and prescriptions

$4,575 to $8,000

American Society of Plastic Surgeons, 2024 national range for the surgeon’s fee alone. Anesthesia, the facility and the implants are extra.

Implants are not lifetime devices, so ask what a future replacement would involve. Read the cost guide.

Providers set their own prices; ask whether a consultation fee applies.

Questions to bring to a consultation

Built from the ASPS list. Write them down and compare the answers.

  1. Are you certified by the American Board of Plastic Surgery?Check it on the ABPS lookup.
  2. Where do you operate, and is the facility accredited?
  3. How many breast augmentations do you perform?
  4. Which shape, size, surface, incision and placement do you suggest, and why?
  5. Will I need a lift as well?
4 more questions
  1. How are complications handled, and what would a revision cost?
  2. How many more implant operations should I expect over my life?
  3. How might this affect breastfeeding?
  4. What exactly does the quote include?

The basics

What breast augmentation is, and is not

Breast augmentation uses implants or your own fat to add volume, restore it after pregnancy or weight loss, or even out a difference. ASPS counted 304,234 in the U.S. in 2025, second only to liposuction.

It does not correct significant sagging: that is a breast lift. Smaller breasts for comfort is a breast reduction, and taking implants out is explant surgery.

The FDA

Risks the FDA describes

Since October 27, 2021 every breast implant has carried a boxed warning, and your surgeon must go over a patient decision checklist with you, which you both sign. The FDA says: “Breast implants are not considered lifetime devices.”

BIA-ALCL is a T-cell lymphoma that the FDA says “appears more often in patients with textured breast implants.” The FDA also says: “Symptoms such as fatigue, memory loss, rash, ‘brain fog,’ and joint pain may be associated with breast implants.” These are still being studied.

The FDA also reports cancers in the scar capsule that it believes may be rare, and calls MRI the most effective way to find a silent rupture. Read Risks and Complications of Breast Implants.
