Breast reduction is the opposite of augmentation, and for many people it is not cosmetic at all. Cleveland Clinic describes it as an operation that removes excess breast tissue and skin, used for large breasts that cause back, neck or shoulder pain. ASPS counted 76,040 cosmetic breast reductions in the U.S. in 2025.
Breast Augmentation NYC is a free referral service, not a clinic or medical practice. A request sent from this page goes to an independent plastic surgery office in New York City, which sets its own prices and is responsible for its own care.
Who asks about a reduction
People usually come to it after years of discomfort. The reasons Cleveland Clinic gives center on pain in the back, neck or shoulders. If that sounds like you, it is worth starting with your own doctor as well as a plastic surgeon, because your doctor’s notes can matter for insurance.
Insurance and pre-approval
Cleveland Clinic says insurance often covers breast reduction when it is medically necessary. Insurers decide from documentation: measurements, a letter about your symptoms, and the amount of tissue the surgeon plans to remove, among other things. Start this step early.
- Ask your insurer what it requires for breast reduction, in writing.
- Keep a record of your symptoms and any treatment you have tried, with your own doctor.
- Ask the surgeon’s office whether it takes your insurance and handles pre-approval.
- Wait for the insurer’s decision before you book.
- Ask what you would pay yourself if the claim is refused.
The operation
Cleveland Clinic says breast reduction is done under general anesthesia and takes about 3 to 5 hours. It can be outpatient, or include one night in the hospital. Ask which applies to you, and plan someone to take you home.
Recovery
| Stage | General range (Cleveland Clinic) |
|---|---|
| Time off work | At least 1 week, some need 2 |
| Strenuous exercise | Not for at least a month |
Your surgeon will give you your own timeline. In New York, think about stairs, a commute that involves carrying bags, and how you will get to follow-up visits in the first weeks.
Scars and risks
Cleveland Clinic lists permanent scars, changes in nipple sensation or its loss, loss of the nipple or of tissue, uneven nipples, and possible inability to breastfeed. It notes that smokers have more complications. Ask each surgeon how often they see these problems and what they do if one happens.
Thinking about a family?
Because a reduction can affect breastfeeding, ask the surgeon how their plan might affect it, and whether to wait.
What breast reduction costs
When insurance covers it, what you pay depends on your plan. For people who pay themselves, ASPS gives a 2024 national range of $7,000 to $12,500 for the surgeon’s fee alone for breast reduction in aesthetic patients, and its 2023 national average was $7,800. Anesthesia, the facility and other costs are extra, and each office sets its own prices. The cost guide shows how to read a quote part by part.
Reduction, lift or augmentation?
These are three different operations. A reduction makes the breasts smaller. A breast lift reshapes them without changing their size. An augmentation adds volume with implants or with fat transfer. Name the change you want, and the surgeon can tell you which operation fits it.
Risks the FDA describes
A breast reduction does not use implants. If you have implants already, or are weighing them, these are the FDA’s points.
On October 27, 2021 the FDA announced new requirements for every breast implant sold in the U.S.: a boxed warning in the labeling, a patient decision checklist that the surgeon must go over with you before surgery and that you both sign, a patient device card, and updated advice on screening for rupture. Sale of implants is restricted to providers and facilities that use the checklist.
- Not lifetime devices. The FDA says: “Breast implants are not considered lifetime devices.” The longer you have them, the more likely complications become, and some complications need more surgery.
- BIA-ALCL. Breast implant associated anaplastic large cell lymphoma is a T-cell lymphoma, a cancer of the immune system, that the FDA says “appears more often in patients with textured breast implants.” It is not breast cancer.
- Other cancers in the capsule. The FDA has reported squamous cell carcinoma and other lymphomas in the scar capsule around implants, with textured and smooth, saline and silicone implants. The FDA believes these may be rare; their cause, incidence and risk factors are not known.
- Systemic symptoms. The FDA says: “Symptoms such as fatigue, memory loss, rash, ‘brain fog,’ and joint pain may be associated with breast implants.” These symptoms, often called breast implant illness, are still being studied.
- Silent rupture. For silicone gel implants, the FDA says MRI is the most effective way to find a rupture that causes no symptoms, and ultrasound is an acceptable alternative.

Questions to ask about breast reduction
- Do you take my insurance, and will your office handle pre-approval?
- What documentation will my insurer want?
- How many breast reductions do you perform?
- Will I go home afterward or stay a night?
- How might this affect breastfeeding?
- If insurance refuses, what would I pay, and what does that include?
Checking the surgeon
Ask whether the surgeon is certified by the American Board of Plastic Surgery (ABPS), and check it on the ABPS lookup. ABPS is a member board of the American Board of Medical Specialties. Then check the surgeon’s New York license on the NYSED Office of the Professions verification search.
