Fat transfer breast augmentation in NYC: the no-implant option

A woman listening as a doctor shows her images on a tablet

Some people want a fuller shape without an implant. Fat transfer breast augmentation, also called fat grafting, uses your own fat instead. It is a real option for some people and a poor fit for others, and the difference comes down to how much change you want.

Breast Augmentation NYC is a free referral service, not a clinic or medical practice. A request sent from this page goes to an independent plastic surgery office in New York City, which sets its own prices and is responsible for its own care.

How fat transfer works

Cleveland Clinic describes it in three steps. Fat is taken by liposuction from another part of the body, purified, and injected into the breasts. It is usually done under general anesthesia as an outpatient operation.

  1. Liposuction takes fat from a donor area chosen with the surgeon.
  2. The fat is purified.
  3. The surgeon injects it into the breasts.

How much change to expect

Cleveland Clinic says fat transfer adds about “one or two cup sizes”. It does not lift breasts that sag. If you hope for a larger change, or for breasts that sit higher, ask about implants or a breast lift as well.

Not all of the fat survives in its new place. Cleveland Clinic says some fat dies, so surgeons inject extra, and more than one session may be needed to reach the size you want. Ask each surgeon how they judge how much has stayed, and when.

Recovery

Cleveland Clinic gives these general ranges. Your surgeon will give you your own timeline.

StageGeneral range (Cleveland Clinic)
Time off workAbout 1 week
Pain and bruisingUp to 3 weeks
Swelling settles, final result showsUp to 6 months

Remember that recovery covers two areas: the breasts and the place the fat came from. Ask what the liposuction side involves, including any garment you wear afterward.

Risks of fat transfer

Cleveland Clinic lists fat necrosis (fat that dies), infection, and cysts or calcifications that may need extra imaging. It says fat transfer does not raise the risk of cancer, and mammograms still work for most people.

Liposuction has risks of its own. Ask the surgeon to go through both parts of the operation with you, and to tell you what they would do if a lump appears.

Who it may suit

From the sources above, fat transfer tends to come up for people who want a modest change, have fat to spare in a donor area, and would rather not have an implant to monitor and later exchange. It tends not to suit someone who wants a large change in one operation, or whose main concern is sagging. Only a surgeon who examines you can say.

What fat transfer costs

ASPS gives a 2024 national range of $5,500 to $9,500 for the surgeon’s fee alone for breast augmentation with fat grafting only, and its 2023 national average was $5,719. That is higher than its range for implants, and the operation includes liposuction as well. Anesthesia and the facility are extra, and a second session would add to it. See the cost guide for how to read a quote.

Risks the FDA describes

Fat transfer uses no implant, so the FDA’s implant warnings do not apply to it. They do apply if you are weighing implants as the alternative, so here they are side by side.

On October 27, 2021 the FDA announced new requirements for every breast implant sold in the U.S.: a boxed warning in the labeling, a patient decision checklist that the surgeon must go over with you before surgery and that you both sign, a patient device card, and updated advice on screening for rupture. Sale of implants is restricted to providers and facilities that use the checklist.

  • Not lifetime devices. The FDA says: “Breast implants are not considered lifetime devices.” The longer you have them, the more likely complications become, and some complications need more surgery.
  • BIA-ALCL. Breast implant associated anaplastic large cell lymphoma is a T-cell lymphoma, a cancer of the immune system, that the FDA says “appears more often in patients with textured breast implants.” It is not breast cancer.
  • Other cancers in the capsule. The FDA has reported squamous cell carcinoma and other lymphomas in the scar capsule around implants, with textured and smooth, saline and silicone implants. The FDA believes these may be rare; their cause, incidence and risk factors are not known.
  • Systemic symptoms. The FDA says: “Symptoms such as fatigue, memory loss, rash, ‘brain fog,’ and joint pain may be associated with breast implants.” These symptoms, often called breast implant illness, are still being studied.
  • Silent rupture. For silicone gel implants, the FDA says MRI is the most effective way to find a rupture that causes no symptoms, and ultrasound is an acceptable alternative.

Questions to ask about fat transfer

  • How much change do you expect for me, and how confident are you of it?
  • Where would the fat come from?
  • How much fat usually survives in your experience, and how many sessions might I need?
  • What would you do if a lump appears?
  • How many fat transfer breast augmentations do you perform?
  • What does the quote include, and what would a second session cost?

Checking the surgeon

Ask whether the surgeon is certified by the American Board of Plastic Surgery (ABPS), and check it on the ABPS lookup. ABPS is a member board of the American Board of Medical Specialties. Then check the surgeon’s New York license on the NYSED Office of the Professions verification search.

Questions people ask

How much bigger can fat transfer make my breasts?

Cleveland Clinic says about one or two cup sizes. Ask the surgeon what they expect for you.

Does all the fat stay?

No. Some fat dies, so surgeons inject extra, and more than one session may be needed, according to Cleveland Clinic.

Does fat transfer lift the breasts?

No. A lift is a separate operation. Read the breast lift guide.

How long is recovery?

Cleveland Clinic says about a week off work, pain and bruising for up to 3 weeks, and swelling settling over up to 6 months.

Will it affect mammograms?

Cleveland Clinic says mammograms still work for most people, though cysts or calcifications may need extra imaging.

Does fat transfer cost less than implants?

Not by the national figures: ASPS gives $5,500 to $9,500 for the surgeon’s fee for fat grafting in 2024, against $4,575 to $8,000 for implants.
